At times, fibrous processes or other causes may determine a mechanical obstructive impediment to an adequate venous return, causing circulatory stasis with a production of varicocele and venous lakes of the cavernous veins, visible with a color Eco-doppler at the penile root, the penis being in a flaccid state. During erection, even in those younger than age 50, the early loss of penile rigidity during coitus can also be related, aside from psychogenic factors, to this veno-occlusive dysfunction known as Venous Leak.
Color Eco-doppler evaluation with the flaccid penis allows identifying at first sight the presence of venous dilations (varicose veins) of different caliber.
The discovery of varicose veins of the cavernous veins at the penile root with dilations greater than 2 mm made it possible to associate them with venous leakage in all those patients presenting with this condition.
In an attempt to correct this situation, the application of focalized low intensity shock waves at the penile roots has resulted in the reduction of the caliber of said venous dilations.
The proposed mechanism of action consisted in incrementing the vascularization at that level, mediated by VEGF, in order to improve the tone and perivenous muscular atrophy, given the poor response obtained by previous venotonic medications.
The finding of varicose veins of the cavernous veins at the level of the penile roots, with dilations greater than 2 mm, could be associated with venous leakage in all patients presenting with this condition.

The results obtained after 4 sessions of 3000 focal waves each (1 to 2 per week), with a frequency between 3 and 4 Hertz and with a power of 0.25 mj / mm2, was the reduction of the caliber of the veins in more than 50% of its original caliber, accompanied by a marked improvement of the symptomatology.
